The three-eyed raven is a raven (with a third eye on it's forehead) that appears to Bran in his dreams. Jojen Reed, who also has green dreams recognises Bran's abilities and suspects the three-eyed raven is an avatar for a green seer that is attempting to contact Bran. Bran et al... set out to find him north of the wall mistakenly believing that he will help Bran to walk again. He is instead the last Green Seer, Brynden Rivers, the former Hand of Aerys I, Lord Commander of the Night' Watch (sent to the wall with his great nephew Maester Aemon) and a commander of Targaryen forces during the Blackfyre Rebellion against his half-brother Bittersteel. Nick-named Bloodraven for the wine stain birthmark on his cheek that resembles a raven, he is an albino with a missing eye. At 126 years old he seeks Bran as a successor.
